Dear District Employees:

It’s that time of the year again!   Open Enrollment 2018 is effective now through Tuesday, October 31, 2017.   The elections you make during Open Enrollment will remain in effect from January 1, 2018 until December 31, 2018 and cannot be changed or revoked until the next annual open enrollment period, unless you have a qualifying life event.  Should you experience a qualified life event, you must notify Benefits Office within 30 days of the qualifying event. 

If you would like to make changes to your current benefit elections you must complete the attached Open Enrollment Form by October 31, 2017.  Instructions on how to complete the Open Enrollment Form are attached to this email.  You must complete an enrollment form if:

Ø  You wish to add/terminate dependents from your medical, dental, prescription or vision coverage

Ø  You are enrolling in medical, prescription, dental or vision for the first time

Ø  You are currently enrolled and wish to waive/ change your medical, prescription, dental or vision benefits

Ø  Enroll or renew participation in the Health or Dependent Care Flexible Spending benefits to pay for all eligible expenses not covered by the medical, dental or vision plan

If you are  waving coverage you must provide proof of other coverage.   When enrolling dependents you are required to provide documentation such as photocopies of birth certificates or passports, Social Security cards, a marriage certificate, the most recently filed tax return Form(s) 1040 and or proof of recent joint debt. 

Open Enrollment Benefit Fair Dates: 

·         Wednesday, October 18, 2017 Harold Wilson School Cafeteria 3:00pm-6:30pm

·         Thursday, October 19, 2017 NPS Central Office Conf. Rm. 914 10:00am-2:00pm

 Representatives will attend  from Aetna Health Insurance Plan, NVA Vision Plan, Dental Plans, Benecard Prescription Plan, Various Tax Sheltered Annuity Companies, Weight Watchers, the Employee Assistance Program and more!

How to Make Enrollment Changes    

Please note, eligible employees who want to make changes, must come to Newark Public Schools Central Office, 2 Cedar Talent Office - Benefit Services, Room 811-A.  If you cannot come into the Benefits Services Office, you can also make changes on the attached fillable Open Enrollment Application attached to this email or on the BenePortal Site.   Instructions on how to complete the Open Enrollment form are attached to this email as well.  

Upon completing the online application, you must hit the “Submit” icon at the top of the form and the application will automatically be emailed to the Benefits Services Office email address.  If you add dependents you can scan copies of your supporting documentation (birth certificates, social security cards, marriage licenses, most recently filed tax return form(s) 1040 for you and your spouse if you filed jointly or both 1040 form copies if you filed separately or proof of joint debt) to the following email address benefits@nps.k12.nj.us.
